The story revolves around Hazel Grace Lancaster, a sixteen-year-old girl with thyroid cancer that has spread to her lungs. She attends a cancer patient support group where she meets Augustus Waters, a seventeen-year-old osteosarcoma survivor. Augustus is charming, witty, and shares Hazel's acerbic sense of humor and disdain for the conventional. Their bond is formed over their shared experiences with cancer and their love for a book called "An Imperial Affliction."

Hazel, who is obsessed with the novel, longs to understand its abrupt ending. Augustus uses his "wish" (granted to him by a charity that grants the final wishes of children with terminal illnesses) to take Hazel to Amsterdam to meet the book's reclusive author, Peter Van Houten. This journey brings them closer and deepens their relationship.
